Filled out affidavite but never got 1099 form? what to do?

Hi i won a grill arv 3,440 filled out the aff and sent it back but still have not recvd the 1099 and want to get my taxes done. I contacted the company but got no reply twice. Any suggestions? Also any way to offset the amount i need to pay or anyone know the percentage? I heard you can go by the fair market value? the prize seems very high based on what I see it online for. First did you received the grill before 1/01/12?

If you did not receive or are still waiting it will go on next year taxes.

If it does go on this year taxes you can contact the JA that handled the sweepstakes for help in gettng a 1099.
Or you can report the income without a 1099.

Yes you can use fair market value
