Ok, got a She Knows win... They are requiring me to fill out an affidavit and send a copy of my drivers license as well for a prize valued at $35.  Really..  Is this just something She Knows always does no matter what the prize... Has anybody else won from them, if so, did you have to fill out the same things as they are asking from me??? I have only been sweeping for a couple months and have won a couple dozen prizes, from $1 to $300 and have not had to fill an affi out yet... scratch

I had two prizes before. One was worth 30 bucks and the other was some nail polish and they wanted me to fill out forms. I never did. It just was not worth my time.

That is something SheKnows does for all their prizes. I've filled them out for 2 $20 prizes from them. Once you get it back, they tend to be quite quick to ship.

Yep. I had to if I wanted wins that were worth as little as $5.

Not worth the trouble and expense, since an affy costs about $10 to get notarized unless I want to drive 45 minutes across town to my Credit Union.  crazy tongue

The sheknows affy doesn't require notorization, or a SSN. It's a fairly light form as far as affy's go... but it does seem like it's a bit much when larger value wins don't require as much info and can be handled strictly in an email exchange.

Filling out a form does not worry me, but how safe is it to submit a copy of your driver's license ,or the last four digits of your social security number?

Well, I am a notary public in Hamilton County, TN, so if any of you are close by, within 20 miles or whatever you are comfortable with, I will notarize your affy for free.  The last time I had something notarized at a bank, they charged $3.  Been awhile, I know.  I even saw a car with a magnetic sign on the side advertising that the driver was a notary public.  I don't know what she charges.
