Heya! Won the $50 Giftcard with a moneyclip! Hope I get it? I got a screen stating I won and it said 'we will need your contact information so we can award you the prize' I clicked the next button and it went to a screen that said 'information received' but I didn't fill anything out! Does anyone remember if this one had info we filled out ahead of time? I totally can't recall. I am a carecredit member so that won't be an issue.
