Received 2 tax forms for 1 trip from Prizelogic and ePrize

I received a tax form from Prizelogic for a trip I won to New York and a week later I received another tax form from ePrize for a different amount but for the same prize to New York. I was trying to find customer service address to ePrize so I left a message on facebook and they were very nice and gave it to me right away but said they are not affiliated with Prizelogic. So I have contacted both companies and hope I can get this resolved. Also, I am very concerned with such a difference in the amount of each tax form, one being $3,700 and the other $5,000 which was the exact amount allowable for the trip. Help! Any sugguestions? Thank you
 
Prizelogic awarded the prize but the travel agent kept talking ePrize all the time even when I responded "Prizelogic" in my emails. It was as if they were the same to her.

Did you win a multi-sponsored trip? That's just odd! Like kleenex is the same as puffs to her?
 
Is it possible that you won 2 different trips?
OR do you have a very common name and everyone got mixed up?
Did you take the trip yet?
They are similar companies with offices in the same cities, but this is a real conundrum!
 
Prizelogic called me on my win to New York. Subway was the sponsor. I made the trip already in September. But when I received the 1099 from prizelogic, a week later I got one for the same trip from ePrize. Prizelogic has disolved their 1099 now and I am left with the one from ePrize so at least now I don't have 2 of them but they would not respond as to why this happened. All that matters to me is that they were quick to fix this for me. Both are very nice to deal with.
