Received Win with no Affidavit

Hi. I received a letter from Arrowhead Promotions & Fulfillment saying I had won a $500 gift card plus a year's supply of Woolite and tips from Stacy London. The letter had space to check which gift card I want. They also enclosed the rules of the contest which state I have to return an affidavit within 14 days, but there was no affidavit! Is this usual? I've only won small things that just showed up in the mail, and this is my first big win. Hope it's real! Telema
 
Arrowhead Promotions is legitimate. If you didn't get a form to fill out with your personal information, you need to contact them as soon as possible. In the meantime, send in a copy of the letter with the gift card you select. It may be that that's all you need to do.
 
Thanks for your information. I'm glad to hear they are legitimate and there IS phone number. I will call them in the morning. I'm so excited!
