It has been a slow two months all I have won is a blooming onion from outback. Then I received this email from JVC

I have attached the affidavit you will need to fill out and sign. Please return by email or mail, all of my information is below. We must receive you response within the next 3 days or you will forfeit your prize and we will draw another winner. Once we have received the affidavit I will send out your KW-AV60 multimedia AV receiver via UPS and provide you with a tracking number. We ask that once you receive your prize, that you will post a picture on JVC Mobile Entertainment's facebook page.
Pretty nice prize, My son's birthday is this month and this will be a perfect gift. they received my affy and called to say it was going out today. thanks SA and JVC
