Strange pop up on screen

Today I was filling out a Cosmo sweeps for a hair dryer (one entry) and when I click on auto fill a screen popped up. I can't quote it word for word, but basically it said that there were two areas on the page for each of the following name, address and phone number and that google would fill each of the same fields with identical info. I could not see any other form on the page, although I have heard that sweeps do this. Has this happen to anyone else? I checked the rules for the contest and did not find anything concerning auto fill or any other type of entry. This just concerns me and I wondered about everyones thoughts on this.

there are sweeps that have more than 1 field that auto-fill could fill all that is saying is there may have been a place asking for info to sign up for something or a place where you could send an e-mail to a friend for them to enter. It is best to see if you can see where auto-fill filled out your info @
