We are in the process of verifying that you are eligible to win. The completed and signed Affidavit and Release must be returned to our office within 30 days from the date of this message or you will automatically forfeit your prize. Once we have verified your eligibility you will be notified with specific details on your prize.
Please be advised, if you are confirmed as a winner, you will be responsible for all federal, state, and local taxes. As required by law, an IRS 1099 form will be prepared in your name and submitted to the IRS for the value of your prize. You will receive a copy.
