I received a 1099 for a sweepstakes prize I won but never received - now what?

Help! HelloWorld never fulfilled (even though I contacted them many times) a sweepstakes prize from a year ago. Yesterday I received a 1099 in the mail for $800! There's no number to contact on the 1099. Any suggestions?

luckynordberg - HelloWorld [formerly ePrize] is usually not the sponsor, they are the JA [Judging Agency] for the sponsor, and if they failed to fulfill a win, try contacting the sponsor directly.

But, be prepared for frustration, since they [sponsor] usually don't know about any promos they had since HelloWorld ran them.

I've had the best results by contacting them [sponsor] on their FACEBOOK page, hth gl!
