So Ridiculous!!
I won $25 from Uber and they wanted
copy of Driver's Licence and signed affidavit.
I told them to count me out for the $25. And, No Thanks!

You do not pay taxes on anything smaller.

Won an ARV: $20 prize once & I contacted them & asked if a notarized affy was really required
[some JA's/Sponsors will have the same requirements for lower tier prizes as they do for $600+ prizes]
when they said yes it was required I turned it down - a notary would have cost me $10!
