Congratulations! You have been selected to receive a prize by registering in the Up Close with Cristina Sweepstakes, administered by ePrize.
You have won a Trip for Two (2) to Miami, Florida to see a Cristina Saralegui show. Trip package includes roundtrip air transportation for two (2) to Miami, Florida; three (3) nights hotel accommodations; two (2) tickets to the Cristina Saralegui show; roundtrip transportation between hotel and concert; and $500. Please see the Official Rules for further prize details and eligibility requirements. This prize has an approximate retail value of $4,500.
In order to receive your prize there are two affidavits that must be completed. The first affidavit is the winners affidavit, named the Affidavit of Eligibility, Liability & Publicity Release (affidavit). It must be completed by you, the prize winner. The person who will be traveling with you must complete the second affidavit, named the Travel Companion Affidavit of Liability & Publicity Release. If you would like your under 18 child to be your travel companion, you will need to complete the Under 18 Traveling Companion form for them. These documents are available online by visiting

Please return the original completed copies, signed and notarized, within ten (10) calendar days of the date of your initial win notification. If you do not return the affidavits within this timeframe, another winner will be selected. You can find the mailing address at the top of the winners affidavit.
You will be notified of your approval status once we have received and reviewed your fully executed affidavits. You will be contacted regarding the fulfillment of your trip shortly after winner verification. Winner must travel on a date designated by Sponsor which must conclude by December 31, 2009, or prize will be forfeited.
Please be advised that there are tax ramifications that will result from your receipt of this prize. If you choose to receive your prize, you will be sent a 1099 form at the end of this calendar year. Please consult a tax advisor for further information about how to best address this issue.
