my wife has had a heart attack and she needs an operation to implant a difib device, we can't afford it, so any help or advice would be helpful. Have tried HRS and SS to no avail.

SSA is notorious for 1st refusal, so research to see if anyone else in a similar position found a way to get heard.
 
When my mom had cancer and needed treatment, she didn't have insurance. She was able to get Ucare (but we live in MN) to pay for it. You might want to check with your county for a social worker to see if they can direct you.
Also, some hospitals (like the one I work at), have programs to assist people.

talk to social services at the hospital and even the drs office sometimes they have some "inside " knowledge to help. sometimes the maker of the device will even donate it if there is a need . Hope this helps
