I just won a 50 inch tv in the Land O frost sweepstakes. The only thing im concerned about is they need an affidavit returned with my social security number and date of birth. Im a little hesitant to send that info to them via fax. ANYONE else run into this?
 
That's pretty standard on larger wins. They need the info to report your win to the IRS. I've filled out several over the years with no problem.

If you entered it and know it was a legitimate sweeps, then it will be fine. If you have a scanner, you can scan the documents and email them instead of faxing. Most sponsors or fulfillment companies request the winner's SS# when prize value is over $600 as it is required for IRS reporting. Some even request it if the prize is under $600 to verify the winner. I won prizes under $600 from Ellen and they requested my SS#. It's not unusual. Congrats on your win and enjoy your new TV!

The threshold for IRS mandatory reporting is $600 & above. For amounts of $599 and below reporting is not required. Some firms prefer to collect the info regardless, that way, they're covered when they take a tax deduction. Like Amy said, ALL sweepstakes winnings are taxable and you're required to report them and pay taxes on them regardless of the dollar amount. Most of us ignore this requirement for wins of $599 or less, since there is usually no 1099 issued. If you're audited & caught you'll be required to pay taxes and penalties on the amount ($200) not reported.
