In today's mail was a box that contained coveralls, eye protection, respirator, rug gripper, rug fastener, earplugs and safety glasses. Inside the box was an affy that said it must be completed and signed before I would become a winner. It is dated July 8, 2013. The amount of the items is $54.00. Since I already got the items should I send back the affy?

I won the same but decided against the affy. After all, we already got the win. And if they really want it they'll contact us I assume...congrats!
 
I received that same prize with an affidavit?
I thought the affidavit is always sent first and then the prize.
The affidavit also needs to be notarized. For such a small prize,
that surprises me.
 
Congrats to each of you. Some wins will surprise you. I remember years back winning a small Crayola Prize (less than $10) and it needed to be notarized. So I just declined that prize.
 
hi everyone, glad you folks posted this - because I rec'd the same prize two days ago with the Affy to be filled out and I'm thinking "eh?" I haven't sent it back- to be honest with you I don't even remember this particular giveaway !
