Congratulations! You have been identified as a potential First Prize winner in the SNICKERS® Brand NFL 2015 Game & Sweepstakes (the “Sweepstakes”) Second Chance Drawing sponsored by Mars Chocolate Sweepstakes & Giveaways North America, LLC.
The First Prize consists of two (2) tickets to a 2016 NFL regular season game (approximate retail value of $300). Please refer to the attached copy of the Sweepstakes’ Official Rules for complete details and restrictions. Please visit NFL 2016 Regular Season Week 1 Schedule - NFL.com to view the NFL regular season schedule.
If you have reached the age of majority in your state of residence, you must complete and sign the enclosed Winner's Acceptance Form, Release and Questionnaire (“Verification Documents”) and have your signature notarized to claim the First Prize. If you have not reached the age of majority, your parent or legal guardian must complete and sign the Verification Documents and have his or her signature notarized.
Please return the completed Verification Documents as soon as possible but no later than ten (10) days after the date above. The Verification Documents will be relied upon to verify your eligibility to receive the First Prize. If we do not receive your completed Verification Documents by July 11th, 2016 you may forfeit your First Prize.
You can return your completed Verification Documents by email ([email protected]) or fax (800-627-3261). If you do not have access to a printer, please let us know via reply email so we can mail the forms to you. Once your completed documents are received and verified we will begin the arrangements for the prize; please note that tickets may not be available to until 4-6 weeks prior to your game choice.
