You have been selected as the winner of a trip for two (2) to The 2009 National Buffalo Wing Festival in Buffalo, NY. The trip package includes roundtrip coach-class air transportation for two (2) from a major airport near winner's home (determined by Sponsor in its sole discretion) to Buffalo, NY, three (3) days and two (2) nights accommodations at the Adam's Mark Buffalo (single room, double occupancy), a SUV-size rental car for length of stay, daily breakfast for two (2), two (2) days admission to the National Buffalo Wing Festival, dinner for two (2) at Anchor Bar, dinner for two (2) at the Buffalo Chop House, and a Niagara Falls Tour for two (2) (Maid of the Mist, Cave of the Winds, and Whirlpool Jet Boat). In order to confirm your eligibility, there are two affidavits that must be completed. The first affidavit is the winner's affidavit, named the Affidavit of Eligibility, Liability & Publicity Release ("affidavit"). It must be completed by you, the prize winner. The person who will be traveling with you must complete the second affidavit, named the Travel Companion Affidavit of Liability & Publicity Release. If you would like your under 18 child to be your travel companion, you will need to complete the Under 18 Traveling Companion form for him or her. Please return the original completed copies, signed and notarized, within ten (10) calendar days of the date of your initial win notification. If you do not return the affidavits within this timeframe, another winner will be selected. You can find the mailing address at the top of the winner's affidavit.
You will be notified of your approval status once we have received and reviewed your fully executed affidavits. You will be contacted regarding the fulfillment of your trip within 8-10 weeks after promotion conclusion.
