Congratulations! You have been chosen as the potential Grand Prize winner in the Texas On Tour Sweepstakes, sponsored by RedPeg Marketing! You entered this sweepstakes online at Texas On Tour Sweepstakes :: Homepage or at a Texas on Tour event kiosk at a recent event/fair you attended.
The Grand Prize consists of a trip for two (winner and one (1) travel companion) to a destination within the state of Texas. Grand Prize winner has the option to choose one (1) of the following four (4) Texas vacation packages: (1) Great Outdoors, (2) Urban Adventure, (3) Beach Getaway or (4) Cowboy Excursion. Trip must be taken by November 30, 2015.
The Grand Prize trip includes: round-trip coach-class air transportation for two (2) from a major U.S. airport closest to winner’s home (at Sponsor’s discretion) to a major airport near the destination city in Texas; six (6) days/five (5) nights’ standard hotel accommodations (one (1) room, double occupancy), including hotel taxes. Approximate Retail Value (“ARV”): $4,000.00. Winner and travel companion must travel together on the same itinerary. Please note that all federal, state and local taxes are the responsibility of the winner.
In order to claim the Grand Prize, you must complete and return the enclosed Affidavit of Eligibility and Liability/Publicity Release Form (“Affidavit”) and W-9 Form. Your travel companion must complete the Liability/Publicity Release Form For Guest (“Release”). All three (3) documents must be completed and returned to me by March 11, 2015. If the Affidavit, Release and W-9 Form are not completed and received at Black Diamond Media by March 11, 2015, you will not be eligible to claim the Grand Prize.
A copy of the Official Rules is enclosed for your reference. Please note the Affidavit and Release need to be signed in the presence of a notary public in order to be valid. Please return the completed Affidavit, Release and W-9 Form to:
