That seems a bit much - I won a prize from the USTA a year or so ago - more expensive tennis bag - and didn't have to provide SS. Not sure if I would for a hat.
 
I have won small prizes that they wanted ss# on and I sent in the paperwork without it. I wrote on it that the value was less than $600 so they shouldn't need it and they sent my prizes anyway. Ezwingame required a notarized form for a $25 Wal Mart card and my bank notorizes for free since I have several accounts there.
 
You could try politely communicating your concern about providing your social security number. It is good that you have the name of a contact person, plus an email address, phone number, fax number and address.

As I read the rules, the potential Grand Prize winner (of the $1,000) would be asked to sign and return the affidavit. I don't see anything in the rules about second or third prize winners being asked to sign affidavits.

"TWENTY TWO (22) THIRD TIER PRIZES: Each consisting of one (1) Official 2010 US Open cap. Approximate Retail Value ("ARV"): $22, each..."

:sunny: Congrats on the win! I would send the form back without the SS# or possibly just make one up! :laughing: Just depends on how bad you want that hat.
 
Congratulations!!! I would send the form back without your soc sec #. Shouldn't be required since the prize is under $600.00.
