CVS Home Team Heroes Contest – a sweepstakes for soccer‑playing families to win incredible community prizes.
How to Enter – During the promotion period, visit CVSHomeTeamHeroes.com, complete the online form and nominate your child by providing:
1. The child’s name.
2. Answers (≤250 words each) to:
- How is your child helping make a difference in the community?
- How has soccer shaped who your child is on and off the field?
- What does being crowned a “Home Team Hero” mean to your child and their community?
3. Upload three recent photos (2 soccer‑action, 1 community‑involvement).
Each complete submission equals one free entry. Entries must be in English and submitted by the nominator (the account holder of the email used). Duplicate accounts or non‑compliant entries will be disqualified. Normal internet/phone charges apply.
Entry Requirements – By submitting, you confirm you are the parent/guardian, have permission to enter on the child’s behalf, and that the entry is original, non‑offensive, and does not infringe any third‑party rights. Entries become the property of CVS® Pharmacy, Inc., which may edit, reproduce, and share them on social media or other channels. Automated or scripted entries are prohibited.
Judging – A panel of Sponsor‑selected judges will evaluate all submissions between September 25 and November 1 using four equal criteria: Leadership, Teamwork & Collaboration, Sportsmanship & Character, and Positive Community Impact. Two Grand Prize winners and two Runner‑Up winners will be chosen. Ties are resolved by the Community Impact score. Judges’ decisions are final.
Prizes and Approximate Retail Values (ARV)
Grand Prizes – 2 winners:
- $25,000 grant payable to the nominee’s soccer organization
- Nominee’s likeness featured on select Well Market packaging
- Signed soccer ball from a Sponsor‑selected athlete (ARV $300)
- A Block Party for the nominee’s soccer team (non‑grant ARV $1,000)
Runner‑Up Prizes – 2 winners:
- $10,000 grant payable to the nominee’s soccer organization
- Signed soccer ball from a Sponsor‑selected athlete (ARV $300)
All grant funds go directly to the winning soccer organization; the nominee and nominator do not receive cash. Grant claims must be signed by an authorized organization representative within the deadline, or the award may be reassigned.
